What
if I can't open up the form?
To
fill out the marriage application online, you first need to make sure your
computer can read the form. The form can be opened using Adobe
Acrobat Reader which is freely available to download on the Internet at
the following site. Just click here
. Just follow the prompts at the site to download Acrobat
Reader. Then the form will open up automatically on your computer
when you click on the application button.
What
if I make a mistake on the form?
When
the form opens up, you will notice the "Clear" button on the top
right corner of the document. This button will reset the form and
erase any items that you have typed in. The button will not print
out on the actual form.
How
do I start to type?
The
first step is to move the mouse cursor to the box where you enter your
name and click once. This will activate the field so that you may
type your name. When you have finished, click on the next box that
you need to fill out and so on. Do not use the tab button on your
keyboard to move from field to field.
Which
fields must I fill out?
You
need only fill out the fields that allow you to type. Therefore, you
must enter all information under the Bride and Groom sections and you must
enter all information under the Record of Divorce section, if
applicable. Then move to the line that names who will be solemnizing
the marriage and fill out the name of the pastor, minister, etc. that will
be performing the marriage.
What
information goes into each field?
Bride
and Groom Section
Name:
Enter the full name of bride or groom, including middle name.
SS#:
Enter your social security number. This will be kept confidential by
the court.
Address:
Enter your current address, including city, state and zip code.
Phone#:
Enter your current phone number where you can be reached.
D.O.B:
Enter your date of birth.
Age
last birthday:
Enter your current age.
Residence:
Enter the county you live in.
Birthplace:
Enter the state you were born in.
Occupation:
Enter your current occupation.
Father's
Full Name:
Enter your father's full name.
Mother's
Maiden Name:
Enter your mother's full name (maiden name).
Previously
Married:
Enter the number of times you were previously married. Also, enter
if you were widowed or divorced from the last spouse. If divorced,
move on to the next section, Record of Divorce.
Record
of Divorce Section
Previously
Married to:
Enter the name of the last spouse.
Divorce
Date:
Enter the date of the last divorce.
Decree#:
Enter the number of the last divorce decree.
Divorce
State/County:
Enter the two digit abbreviation for the state in which your last divorce
was granted, and then enter the county.
Minor
Children:
Enter all minor children of previous marriages.
Do
not fill in the Consent of Parents section, this will be signed at the
Courthouse in front of a deputy clerk.
Enter
the name of the person solemnizing the marriage in the paragraph following
the Consent of Parents section.
What
do I do after I've completed the form?
After
checking the form for accuracy, print it out, and bring it to the Probate
Court along with the other required materials and your license will be
issued. |
